Additional Forms, Guides and Workbooks

  • Philmont Leader Application: This is the leader application form for Philmont. We will need to submit this, along with $250 -- credit card seems likely -- to the Council with respect to each adult wanting to go with the boys for the 2003 summer.
  • Philmont Scout Application: This Form has been left here FYI. It is indeed the 2003 Application Form and has no relevance for the 2003-2004 season.

  • Individual History Transfer Report: Use form to record corrections to the Individual History Report. Forward form to Carol Burger. (Temporarily Unavailable)
  • Personal Health and Medical Record class 3: All Class 3 activities require a health examination within the past 12 months by a licensed health-care practioner. This includes youth and adult members participating in high-adventure activities, athletic competion, and world jamborees. Annually, this form is to be used by adults over 40 for all activities requiring a physical examintaion and applies to all Wood Badge participants regarless of age.
  • Forms - NCAC (Council): All forms provided by our Council, National Capital Area Council.

Eagle Scout Forms, Guides and Workbooks

  • Eagle Applications: This application is to be completed after you have completed all requirements for the Eagle Scout rank.
  • Eagle Scout Leadership Project Service Workbook:You may proceed with your leadership service project only when you have completed all the planning details mentioned herein; shared the project plans with the appropriate persons; obtained approval from the appropriate persons.
  • Eagle Scout Service Project Worksheet: This document is the LIFE TO EAGLE GUIDELINES workbook, Part 1, Published by South Florida Council, 1995. The forms included here cover pages 23-42 of that publication. (Under repair)
  • National Eagle Scholarship Application Form: Please be sure to use only this form when applying. Application forms with a printing date before 1994 are no longer usable. To be sure you are using the correct form, call the National Eagle Scout Service, 972 580-2431.
  • Selecting Your Eagle Leadership Service Project: This document is from the Marin Council, CA and is information that the Marin Council provides to Eagle candidates. For our purposes, the contacts, etc, obviously need to be from Banneker District, but the thrust and details noted will provide you with good advice to get your project approved with a minimum of bureaucratic snarls. Read this early as it will help you to identify service projects in your neighborhood. Read it again and it will help you analyze how to turn your choice into a successful service project.
